    Targeted Muscle Reinnervation (TMR) in the Popliteal Fossa

    This video depicts the Targeted Muscle Reinnervation or TMR procedure, which is indicated for patients with painful neuromas or phantom pain following amputation of an extremity. This procedure first involves the denervation of the targeted muscle followed by the coaptation of a proximal donor nerve to distal motor recipient nerve branches near the motor end plate of that muscle.

    In this case, the patient is a 70 y/o Caucasian female with a PMH significant for a left below-the-knee amputation (BKA). The patient is complaining of persistent phantom pain but denies any neuroma pain. Assessment of the patient reveals a painful L BKA, possibly from a fibular overgrowth, and persistent phantom pain. And the plan is to perform a revision of the L BKA stump with a targeted muscle reinnervation (TMR).
