Two-Portal Endoscopic Carpal Tunnel Release

The dual-portal endoscopic carpal tunnel release technique was originally described in 1987 by Chow, with subsequent modification by Brown in 1992. This remains our preferred technique for primary surgical treatment of carpal tunnel syndrome. Literature over the past decades has repeatedly demonstrated it to be safe and effective, with significantly better outcomes in pain, grip strength, and scar complication rates when compared with open carpal tunnel release. The technique does carry a higher incidence of transient nerve injury, but long-term complication rate is equivalent to open release. This video is a demonstration of the two-portal endoscopic carpal tunnel release according to the modified Chow technique.
