Mini-open Subpectoral Biceps Tenodesis With Onlay All-Suture Anchor Technique

Subpectoral biceps tenodesis can be an effective treatment for patients with pain and debility from pathology of the long head of the biceps tendon in whom conservative treatment has failed. Tenodesis can be performed with a variety of approaches and implants. The open approach may have certain advantages over the arthroscopic approach, but its more invasive nature may increase the risk of complications. Moreover, certain implants such as interference screws may increase the risk of iatrogenic humerus fracture, whereas other implants such as all-suture anchors have noninferior biomechanical properties. Therefore, we present a mini-open technique for subpectoral biceps tenodesis. It uses an onlay all-suture anchor through a 2-cm incision. The manipulation of soft tissue is minimized, and drilling occurs in a unicortical fashion, reducing the risk of axial nerve injury and iatrogenic fracture.
