    Bilateral Needle Arthroscopy–Assisted Plantaris Tendon Release

    Background Although often seen as a vestigial structure, the plantaris can be a source of pain. Pain generated from the plantaris muscle can manifest as noninsertional Achilles tendon tendinopathy or as posterior knee pain at the site of the muscle belly. Needle arthroscopy provides a safe and minimally invasive method of treating the pathology through a release of the plantaris tendon. Purpose This video overview and case presentation demonstrates bilateral plantaris tendon releases for the treatment of plantaris pain. Methods The anatomy, examination, diagnosis and treatment options for plantaris pain are reviewed. Surgical indications and considerations are discussed. A case of a 37-year-old woman who had posterior knee pain and MRI evidence of edema in the plantaris myotendinous junction is presented. After failure of nonsurgical treatment, a decision was made to proceed to surgery for pain relief. Results Minimally invasive complete release of bilateral plantaris tendons was performed by using a needle arthroscope. Postoperative clinical follow-up demonstrated complete resolution of posterior knee pain. Conclusion Plantaris pain can be effectively treated with a minimally invasive approach via a needle arthroscope.
