Roux-Goldthwait Procedure and Medial Patellofemoral Ligament Reconstruction in a Skeletally Immature Female for Recurrent Patella Dislocation

Recurrent patella dislocations are a complex condition in skeletally immature patients. Many treatment options are available, with complications ranging from loss of range of motion to unsuccessful repair. The novel technique discussed in this video combines a modified Roux-Goldthwait procedure and medial patellofemoral ligament reconstruction for the correction of recurrent patella dislocations. This combined technique addresses proximal and distal problems with the use of new and old techniques.

This video discusses the case presentation of a 12-year-old skeletally immature girl who can voluntarily laterally sublux/dislocate her patella by contracting the lateral quadriceps. The patient was previously treated via a guided-growth procedure for bilateral genu valgum and patellar realignment. The procedure shown in this video includes proximal patella realignment via medial patellofemoral ligament reconstruction and distal realignment via a modified Roux-Goldthwait procedure. Gentle active range of motion was encouraged immediately postoperatively, and the patient underwent aggressive physical therapy that began 4 weeks postoperatively. The patient had full range of motion and a stable patella at 3 months postoperatively.
