Acute Compartment Syndrome of the Extremity
Introduction: Acute compartment syndrome is well known among orthopaedic surgeons. The timely diagnosis and management of acute compartment syndrome is crucial to preventing its sequelae, which include renal failure, ischemic contracture, and limb loss. Despite its relative importance, acute compartment syndrome is a challenge for many residents and clinicians because diagnosis largely relies on clinical judgment.
Methods: Timely diagnosis and thorough compartment release are essential to optimize the outcomes of patients with acute compartment syndrome. This video highlights a clinical case study of a patient in whom compartment syndrome of the leg was suspected, diagnosed, and surgically managed.
Results: This video discusses the indications for compartment release and demonstrates compartment assessment with the use of an arterial line transducer, four-compartment fasciotomy via two incisions, and temporizing vessel loop closure.
Conclusion: Acute compartment syndrome may be a devastating complication of common fractures. Orthopaedic practitioners must understand the immediacy of management. A timely, accurate diagnosis and expedient surgical management are essential for patients with acute compartment syndrome.
