Distal Radius Fractures Open Reduction Internal Fixation: Case Presentation and Surgical Technique
Purpose: Distal radius fractures are commonly encountered in the emergency department, occurring in a bimodal age distribution in younger males with high-energy injuries and older women who sustain low-energy fragility fractures. Treatment can be divided into closed reduction and casting and surgical fixation the latter of which is one of the most commonly performed orthopaedic procedures. This video demonstrates fixed-angle volar plate fixation of distal radius fractures and reviews the indications for surgical fixation.Methods: Various devices and techniques have been developed and described for surgical fixation of distal radius fractures. Options include closed reduction with percutaneous fixation and open reduction and internal fixation. The use of fixed-angle volar plates and a volar Henry approach effectively restore the native anatomy.Results: This video presents a case of a distal radius fracture reduced using a fixed-angle volar plate. We demonstrate the volar Henry approach for open reduction and internal fixation of a dorsally angulated distal radius fracture. Anatomic reduction is achieved with the judicious use of fluoroscopy to align and fixate the fixed-angle volar plate.Conclusion: Studies have suggested advantages of surgical fixation of distal radius fractures over nonsurgical management. Surgery is the most reliable method to restore normal anatomy. A review of current indications is important for the modern orthopaedic surgeon.
