This video describes the indications and technique for reconstruction of the labrum in patients who have irreparable tears. Hip arthroscopy has evolved substantially during the past decade, and improved outcomes have been noted with labral repairs. However, not all ace tabular labral tears are reparable. Dr. Dominic Carreira has developed a shuttle technique using allograft fascia lata tissue from the University of Miami Tissue Bank for reconstruction of the labrum. In addition to the treatment of associated pathologies in the hip, the shuttle technique includes resection of the damaged labrum, creation of an accessory distal portal, and delivery of the graft into the joint after suturing the ends of the graft outside of the joint. Rehabilitation after labral reconstruction is similar to that of labral repair surgery. In addition to complications associated with standard arthroscopic labral repair surgery, allograft tissue incorporation failure is a concern, although not noted to date. We are currently conducting a minimum 2-yearfollow-up study on patients who have had allograft labral reconstruction surgery. The initial 20 patients had a mean modified Harris hip score of 85, and all but one patient were satisfied with the surgery and say they would have it again.
