It is the viewer’s responsibility to determine the educational value of this historical content.INTRODUCTION: Full thickness cartilage injuries of the knee are a common therapeutic challenge for the orthopedic surgeon, especially when the pathology is in the patella. A common treatment is an osteochondral autograft transfer in which an osteochondral plug is transferred from an area of less contact pressure to the focal chondral defect. Even though this technique can be performed at the patella, results are not as encouraging as those observed in the femoral condyles. OBJECTIVE: The goal of this video is to describe the surgical technique of an osteochondral autograft transfer system for isolated chondral lesions of the patella. Of course, patient selection and adequate surgical technique are fundamental to a successful surgery. In addition to the technique, this video describes surgical tips, indications, functional outcomes, and the postoperative management of this procedure.
