Patellar Tendon Augmentation with Hamstring Tendon Autograft

It is the viewer’s responsibility to determine the educational value of this historical content.When patellar tendon ruptures disrupt the extensor mechanism, surgery is the treatment of choice. Acute ruptures are managed using transosseous tunnel or suture anchor fixation. Chronic patella tendon ruptures (those more than 6 weeks out from injury) present a challenge secondary to scar tissue formation, calcium deposition, and fibrous adhesions. Several grafts can be used for reconstruction in cases of chronic patellar tendon ruptures. Allograft options include allograft Achilles tendon and bone-patellar tendon-bone. Autograft options exist as well, with the semitendinosus the most commonly used hamstring tendon. Case reports show encouraging results for reconstruction with hamstring autografts. In this video, we present a case of chronic patellar tendon rupture augmentation using a hamstring tendon autograft. We conclude that hamstring tendon autograft can effectively reconstruct the patellar tendon in cases of chronic patellar tendon rupture.
